While every effort has been made to ensure the quality of services offered, you may be dissatisfied with the services received. If such is the case, you may report your dissatisfaction to the person responsible for the service concerned or contact the Local Service Quality and Complaints Commissioner directly by calling 819 627-9494.
If your dissatisfaction relates to ambulance services, a community health and social services organization, or a private assisted-living facility in the region, you must direct your complaint to the Regional Service Quality and Complaints Commissioner by calling 1 819 764-3264, ext. 49301 or 1 888 764-5531 (toll free).
The function of the medical examiner is to examine all complaints filed by a user or any other person in relation to a physician, dentist, or pharmacist, as well as a resident of the institution, as set out in the Act.
