In-Home Support Services
These services are designed for people of all ages who are experiencing temporary or permanent loss of autonomy, the origin of which is physical, mental, or psychological, as well as for those nearing end of life.
Professionals who provide in-home support services rely on interdisciplinarity (nurses, social workers, physiotherapists, nutritionists, etc.) and collaboration, keeping their patients at the centre of their activities and providing quality care and services. The education and information provided to care recipients and their loved ones are also an integral part of the services provided by this team.
Fall Prevention Program
- This program is designed for individuals 55 and over, living at home, or receiving in-home support services. Its purpose is to raise awareness on the prevention of falls in the home and to adopt safe behaviours. In addition to prevention, this program has two other purposes: education and support for minor home adaptations.

Nursing Care
- Home care nurses mainly provide services to the following individuals:
- persons with loss of autonomy requiring home monitoring
- persons requiring care during a post-op or post-hospitalization period and who are unable to leave their home to receive the treatment prescribed
- persons requiring palliative care
External Resources
- A number of community organizations offer complementary in-home support services. Domestic help, meals-on-wheels, and assistance are provided by “Independance 65+”.
